If you require the reader to view the PDF file, it can be obtained either through Skyplan at Adobe Reader or directly from the Adobe Website.

When you open up a PDF file from your web browser you should notice that the document opens with its own Acrobat Reader toolbar, including, for example, print, save and zoom icons. Always use the Acrobat Reader toolbar for tasks specific to PDF documents.

Adobe Acrobat is required to save the document with the filled-in form fields for reference or for emailing the file, Acrobat Reader can only save the form with empty fields to be filled in later or provide the ability to fill in the fields and print.

You may either download the file to complete at a later time and then print, sign and return via fax or mail, or you may open the file immediately to complete the form and then you can print, sign and return the form visa vi fax or mail.
